Mountain Laurel Family Practice is pleased to announce that we have received Patient Centered Medical Home Certification Level 3 from NCQA. The medical home is best described as a model or philosophy of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. It is a philosophy of health care that encourages providers and care teams to meet patients where they are, from the most simple to the most complex conditions. It is a place where patients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. Above all, the medical home is not a final destination instead, it is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patient's needs.

Mountain Laurel Family Practice has coordinated a collaborative care agreement with Daymark Recovery Services. Expectations for sharing information, and safe, coordinated, efficient patient care, are set under this agreement. Daymark provides 24/7 availability for appointments by walk-in or referral.

Mountain Laurel Family Practice has a care coordination collaboration agreement with Daymark Recovery Services. This agreement sets the expectations for patient information sharing between services as well as provides safe, efficient, coordinated care. Daymark offers 24/7 availability for patients through setting appointments by referral or walk-in. Walk-in appointments are encouraged.

Urgent Care Co-Management
Mountain Laurel Family Practice, along with AppUrgent Care, has agreed to provide acute and routine care to our patients after hours. The protocols in place allow for AppUrgent Care to have access to information from our patient's records when need be. Also, when necessary, patient records are accessible via NCHIE. Make sure you tell AppUrgent Care that you are a patient at Mountain Laurel Family Practice so that your care can be coordinated appropriately.
After hours, you can reach an on-call physician at (828)265-5505 or you may opt to visit AppUrgent Care.
App-Urgent Care hours are as follows:
Monday - Friday: 8AM- 7PM and
Saturday - Sunday: 9AM - 5PM
